What is Stroke Association Connect?
Surviving a stroke is just the start of someone’s recovery journey.
When your patients are discharged from hospital, within a few weeks, we’ll get in touch to see how they're getting on.
Our friendly Stroke Support Team will provide:
- An outreach phone call to answer any immediate questions or concerns at the start of someone’s recovery.
- Connection to ongoing support based on things people tell us, which can be accessed in the weeks, months and even years after a stroke.
- Practical information about stroke and support to access other local and community services. As well as regular follow-on information, via email or post, to support recovery.
- Essential reassurance to let people know they don’t have to face recovery alone.
- Connection to people who have had a stroke. Including:
- Local and online groups and a vibrant online community. People can ask questions, share recovery tips, and learn from each other to find new ways of doing things.
- People can also talk to someone who’s been through the recovery journey or join an online activity to connect with others and take the next step in their recovery.
- The offer of additional outreach calls for people who need further support.
Your patients can only benefit from this service if you refer them at the point of discharge.
