Stroke affects everyone differently. One of our Stroke Support Coordinators will get in touch to book in your Post-Stroke Review. Your review is a chance to reflect on the progress you have made since having a stroke. It’s a chance for you to talk about things which are important to you and anything you may want support with as you continue your recovery journey.
Who is this service for?
We’re here to support anyone who’s had a stroke. This formal check-in happens 4-8 months after a stroke to discuss your support needs and anything you want to achieve in the future. It’s also known as a six-month review.

- Information about stroke and access to other local services.
- Information on how to reduce your risk of stroke.
- A home visit and/or telephone call and online support.
